Every plan is personalized

NPCCI strives to fit each direct care plan to the person, not the person to the plan. Because we work for the client and not the state or an agency, we are unbiased in our assessments and advocate for what's best for our customer - not what's best for the system.

3. Then we develop a care management plan that addresses the care recepient's health, well-being, and issues of daily living.

The choice is yours

Once your care management plan is developed, you are free to decide how it is implemented. If you prefer to contract with recommended care providers yourself, you may negotiate with them and hire support staff directly. If you would rather have NPCCI manage and coordinate the delivery of your direct care services, you may negotiate a contract with us, instead.

Whichever option you choose, NPCCI will always be there for you. As providers of direct care services, we will effectively implement, monitor and evaluate every aspect of your care plan. In so doing, we will make certain that services remain appropriate as daily living needs change. As your direct service resource, we will advocate for you and your family whenever there is a need.
